    NASAwide electronic publishing system: Prototype STI electronic document distribution, stage-4 evaluation report

    This evaluation report contains an introduction, seven chapters, and five appendices. The Introduction describes the purpose, conceptual frame work, functional description, and technical report server of the STI Electronic Document Distribution (EDD) project. Chapter 1 documents the results of the prototype STI EDD in actual operation. Chapter 2 documents each NASA center's post processing publication processes. Chapter 3 documents each center's STI software, hardware, and communications configurations. Chapter 7 documents STI EDD policy, practices, and procedures. The appendices, which arc contained in Part 2 of this document, consist of (1) STI EDD Project Plan, (2) Team members, (3) Phasing Schedules, (4) Accessing On-line Reports, and (5) Creating an HTML File and Setting Up an xTRS. In summary, Stage 4 of the NASAwide Electronic Publishing System is the final phase of its implementation through the prototyping and gradual integration of each NASA center's electronic printing systems, desktop publishing systems, and technical report servers to be able to provide to NASA's engineers, researchers, scientists, and external users the widest practicable and appropriate dissemination of information concerning its activities and the result thereof to their work stations

    Consultant-led UK paediatric palliative care services: Professional configuration, services, funding

    Objectives: To systematically gather information on the professional team members, services provided, funding sources and population served for all consultant-led specialised paediatric palliative care (SPPC) teams in the UK. Methods: Two-part online survey. Results: Survey 1: All 17 medical leads from hospital-based or hospice-based SPPC teams responded to the survey (100% response rate). Only six services met the NICE guidance for minimum SPPC team. All services reported providing symptom management, specialist nursing care, end-of-life planning and care, and supporting discharges and transfers to home or hospice for the child's final days-hours. Most services also provided care coordination (n=14), bereavement support (n=13), clinical psychology (n=10) and social work-welfare support (n=9). Thirteen had one or more posts partially or fully funded by a charity. Survey 2: Nine finance leads provided detailed resource/funding information, finding a range of statutory and charity funding sources. Only one of the National Health Service (NHS)-based services fully funded by the NHS. Conclusions: One-third of services met the minimum criteria of professional team as defined by NICE. Most services relied on charity funding to fund part or all of one professional post and only one NHS-based service received all its funding directly from the NHS

    Examination of the Cytotoxic and Embryotoxic Potential and Underlying Mechanisms of Next-Generation Synthetic Trioxolane and Tetraoxane Antimalarials

    Semisynthetic artemisinin-based therapies are the first-line treatment for P. falciparum malaria, but next-generation synthetic drug candidates are urgently required to improve availability and respond to the emergence of artemisinin-resistant parasites. Artemisinins are embryotoxic in animal models and induce apoptosis in sensitive mammalian cells. Understanding the cytotoxic propensities of antimalarial drug candidates is crucial to their successful development and utilization. Here, we demonstrate that, similarly to the model artemisinin artesunate (ARS), a synthetic tetraoxane drug candidate (RKA182) and a trioxolane equivalent (FBEG100) induce embryotoxicity and depletion of primitive erythroblasts in a rodent model. We also show that RKA182, FBEG100 and ARS are cytotoxic toward a panel of established and primary human cell lines, with caspase-dependent apoptosis and caspase-independent necrosis underlying the induction of cell death. Although the toxic effects of RKA182 and FBEG100 proceed more rapidly and are relatively less cell-selective than that of ARS, all three compounds are shown to be dependent upon heme, iron and oxidative stress for their ability to induce cell death. However, in contrast to previously studied artemisinins, the toxicity of RKA182 and FBEG100 is shown to be independent of general chemical decomposition. Although tetraoxanes and trioxolanes have shown promise as next-generation antimalarials, the data described here indicate that adverse effects associated with artemisinins, including embryotoxicity, cannot be ruled out with these novel compounds, and a full understanding of their toxicological actions will be central to the continuing design and development of safe and effective drug candidates which could prove important in the fight against malaria

    End of life care for infants, children and young people (ENHANCE): Protocol for a mixed methods evaluation of current practice in the United Kingdom [version 1; peer review: 2 approved]

    Background: Although child mortality has decreased over the last few decades, around 4,500 infants and children die in the UK every year, many of whom require palliative care. There is, however, little evidence on paediatric end-of-life care services. The current National Institute for Health and Care Excellence (NICE) guidance provides recommendations about what should be offered, but these are based on low quality evidence. The ENHANCE study aims to identify and investigate the different models of existing end-of-life care provision for infants, children, and young people in the UK, including an assessment of the outcomes and experiences for children and parents, and the cost implications to families and healthcare providers. Methods: This mixed methods study will use three linked workstreams and a cross-cutting health economics theme to examine end-of-life care models in three exemplar clinical settings: infant, children and young adult cancer services (PTCs), paediatric intensive care units (PICUs), and neonatal units (NNUs). Workstream 1 (WS1) will survey current practice in each setting and will result in an outline of the different models of care used. WS2 is a qualitative comparison of the experiences of staff, parents and patients across the different models identified. WS3 is a quantitative assessment of the outcomes, resource use and costs across the different models identified. Discussion: Results from this study will contribute to an understanding of how end-of-life care can provide the greatest benefit for children at the end of their lives. It will also allow us to understand the likely benefits of additional funding in end-of-life care in terms of patient outcomes

    The development and initial validation of The Cognitive Fusion Questionnaire

    Acceptance and Commitment Therapy (ACT) emphasizes the relationship a person has with their thoughts and beliefs as potentially more relevant than belief content in predicting the emotional and behavioral consequences of cognition. In ACT, ‘defusion’ interventions aim to ‘unhook’ thoughts from actions and to create psychological distance between a person and their thoughts, beliefs, memories and self-stories. A number of similar concepts have been described in the psychology literature (e.g. decentering, metacognition, mentalization and mindfulness) suggesting converging evidence that how we relate to mental events may be of critical importance. Whilst there are some good measures of these related processes, none of them provides an adequate operationalization of cognitive fusion. Despite the centrality of cognitive fusion in the ACT model, there is as yet no agreed measure of cognitive fusion. This paper presents the construction and development of a brief, self-report measure of cognitive fusion: The Cognitive Fusion Questionnaire (CFQ). The results of a series of studies involving over 1800 people across diverse samples show good preliminary evidence of the CFQ’s factor structure, reliability, temporal stability, validity, discriminant validity, and sensitivity to treatment effects.     Post-Arterial Filter Gaseous Microemboli Activity of Five Integral Cardiotomy Reservoirs during Venting: An In Vitro Study

    During a previously published study on gaseous micro-emboli (GMEs) and perfusionist interventions, it was noted that emboli could be detected after the arterial filter when blood/air challenges entered the membrane oxygenator’s integral cardiotomy. The findings indicated that further study into the oxygenator’s integral cardiotomy reservoir was warranted. This is the first know published report that connects the vent return to GME activity after the arterial filter. To study the air handling ability of the membranes integral cardiotomy, an in vitro study was conducted on five hard shell coated membrane oxygenators (Terumo Capiox SX25, X coated; Sorin Synthesis, phosphorylcholine coated; Gish Vision, GBS coated; Medtronic Affinity NT, trillium coated; Maquet Quadrox, bioline coated). The oxygenators were matched with their own manufacturer’s coated arterial filters (Medtronic 351T Arterial Filter, Sorin Synthesis Integrated Arterial Filter, Terumo CXAF200X Arterial Filter, Gish GAF40GBS-2 Arterial Filter, and Maquet Quart HBF140 Arterial Filter). There were three arms to the study, and three separate oxygenator/filter combinations were used in each arm. The first arm consisted of a pump flow of 4.0 L/min with only the filter purge blood entering the integral cardiotomy. In the second arm, 500 mL/min of simulated vent blood was added to the filter purge blood entering the integral cardiotomy. During the final arm, 200 mL/min of air was added to the vent blood as it entered the integral cardiotomy, to more closely simulate vent return during cardiopulmonary bypass. All GME activity in the oxygenator/filter combinations was examined using the Hatteland CMD20 Microemboli Counter. Placement of the Hatteland probes was 4 in after the hard shell reservoir outlet (PRO) and 12 in after the arterial filter (PAF). When vent blood flow was turned on, there was a significant increase in the PRO microemboli activity detected in all reservoirs. In the PAF position, three of the oxygenator/filter combinations were able to remove 98–99% of the GME, one removed 84.3%, and another removed only 55.5% of the GMEs coming out of the oxygenator’s reservoir. All oxygenators were found to have a dramatic increase in reservoir GME activity when the vent was turned on. Depending on the oxygenator/filter combination, vent return into the oxygenator’s integral cardiotomy resulted in the presence of significant amounts of GMEs after the arterial filter

    2018 Queensland NAPLAN review: School and system perceptions report and literature review

    This Executive Summary presents main findings from the 2018 Queensland NAPLAN Review Phase 2: School and System Perceptions—Report and Literature Review conducted from August 2018 to October 2018. Drawing on views provided by Queensland participants from Government, Catholic and Independent school sectors, this report provides evidence regarding the “optimal positioning of NAPLAN in the future of education in Queensland” and presents findings related to “any changes needed to address issues raised and improve Queensland’s education system outcomes” (Department of Education Queensland Government [DoE], 2018). This report also looks at the contribution NAPLAN has made in “enabling Queensland students to reach their full potential and the role NAPLAN plays in school and system improvement” (DoE, 2018). The report builds on Phase 1 of the 2018 Queensland NAPLAN Review, conducted separately to gain parent perspectives with respect to NAPLAN through focus groups in three regions and a state-wide online survey (Matters, 2018). The authenticity of this report is grounded in the views of the profession and those who work closely in varying capacities as part of the education system. Researchers at the Institute for Learning Sciences and Teacher Education (ILSTE), Australian Catholic University, were engaged to undertake a Literature Review, and to report the professional views of all participants, through online surveys of Queensland teachers, principals, and school students from Years 3 to 10 on the role of NAPLAN, their NAPLAN experiences, and system improvement. In addition, identified key education stakeholders participated in the Review through the platforms of focus groups, interviews and an organisation survey
